Neighborhood Health Partners offers two main types of service:

  1. Community Health Case Management

  2. Reproductive Healthcare support

Community Health Case Management

  • Neighborhood Health case managers maintain a network of relationships with healthcare services throughout southwestern Wisconsin. Think of it as your “one-stop-shop” of information and referral to healthcare resources to connect you with the services in your community that you may be interested in, or that you didn’t know were available to you.

  • Our case managers are not clinical service providers. We don’t take your temperature.  We don’t give shots.  Our case managers are available to help connect you with the healthcare services that you may need.

  • At present, issues related to COVID-19 are the primary concern for our case managers. Concerned about COVID-19 vaccination?  A case manager can provide up-to-date, unbiased information.

Reproductive Healthcare Support

Neighborhood Health Partners offers a confidential, affordable clinic setting where people of any lifestyle or background can get information about reproductive healthcare, basic tests of reproductive health, healthcare resources, and, if you want, referral to an appropriate specialist.

In clinic and telehealth services are offered Monday through Thursday. Call for an appointment.

  • Annual exams

  • Sports physicals

  • Birth Control

  • STD testing, treatment and partner treatment

  • Cervical cancer screening

  • Breast cancer screening

  • Pregnancy Testing & Support

  • Condoms & Emergency Contraception

What if I have limited ability to pay?

The services of Neighborhood Health Partners are available to anyone.

  • Depending on your income, you might take advantage our sliding-scale fee option.

  • Or, sign-up for our Family Planning Only services. Family Planning Only is a free insurance plan available to those that are income eligible.  This insurance can pay in-full for appointments and certain medications.
